CalEvans In "Truth, Lies, and O-Rings”, Allan MacDonald gives a first-hand account of NASA’s tight schedule/goals, and subjective disregard for safety. Indeed there were lots of problems, but “we must keep this now-impossible schedule, so we must ship” was tragically expensive that day.
